Investigating Accidents

Description

Could you be asked to investigate an accident that happened in your department? Would you know how to go about it? And, having determined the causes, minimise the chances of it happening again? Anyone who may be asked to investigate an accident is welcome, but it is particularly relevant to those who supervise and manage others and Departmental Safety Officers. Video, case studies and discussion will be used to clarify why accidents and incidents should be investigated and how to tackle an investigation. We will look beyond the immediate causes of accidents to root causes.
