- The Accident, Dangerous Occurrence and Incident form, the process of reporting and reasons for the revisions.
- The information required for reporting, subsequent processing and utility.
Objectives
- To provide new and existing DSOs and Administrators with the updated accident and incident reporting forms and to explain the process of reporting in relation to changes in the relevant regulations.
- To provide an understanding of the information required by the Safety Office.
- To provide the context of the broad range people in receipt of some of the information within the University e.g. insurance office, data protection office, estate management.
